**Please note that the above forms are required in hard copy with player and parent signatures. Please turn these forms in at registration, or to your coach or a board member and/or mail them in prior to season start. Also, make sure to print out or forward the email confirmation of your US Lacrosse membership to firstname.lastname@example.org or bring a photocopy of your US Lacrosse membership card to registration. US Lacrosse membership is required and must be kept on file.
