Player & Parent/Guardian Commitment

I understand that Volunteers are a vital part of the club’s success. Each parent/family is required to serve 15 hours of volunteer time for the club. There are many opportunities to complete these hours: jarring salt, selling salt, setting up/tearing down booths, field maintenance, cleaning field buildings, organizing events, etc.

Payment

I agree to 15 hours of volunteer hours & will pay a $75 registration fee.

I waive the volunteer option and will pay the $300 registration fee.

Membership

I agree to purchase a US Lacrosse Membership. This membership includes injury related insurance.

Consent and Agreement

I do hereby consent and agree that North Olympic Peninsula Lacrosse Club or agents have the right to take and use photographs, videotape, or digital recordings of me and to use these in any and all media, now or hereafter known, and exclusively for the purpose of promoting the club and the sport of lacrosse. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.
